If you are a board-certified neurologist, a board-certified neurosurgeon, or other board-certified neuro-specialist physician and would like to join this select group of doctors, click here for the Network Provider Application. Complete the application and send it to the Claims Administrator with the supporting documents described on page one. After receiving your application, the Claims Administrator will review your education, training, clinical experience, licensing, board certification, insurance coverage and other background information to see if you qualify to be a physician in the Program. If the Claims Administrator determines that you qualify, Class Counsel and Counsel for the NFL Parties must approve your participation.
